Vilnius University Hospital Santaros Klinikos Centre of Abdominal surgery provides service for over 11000 outpatients and 3300 inpatients yearly; 3500 surgical operations, 600 interventional procedures, 1200 diagnostic procedures are performed each year. Before surgical procedures all patients are thoroughly investigated using modern imaging modalities (endosonoscopy, CT, MRT), laboratory tests (cancer, inflammatory markers, hormones, oncogenes), examined in pelviperineology, functional gastrointestinal and colorectal laboratories and discussed in multidisciplinary meetings. Most of the diseases are treated using minimally invasive approach – laparoscopy is used for treatment of calculous cholecystitis, obesity, inguinal and postoperative hernia, gastroesophageal reflux disease, achalasia, obstipation, diseases of pancreas and spleen, colorectal tumors, endocrine diseases of thyroid, adrenal, parathyroid glands. Some of above mentioned procedures are performed endoscopically via single incision for better cosmetic effect (cholecystectomy, appendectomy, thyroid, adrenal glands surgery). Narrowings, caused by tumors, are dilated using stents to improve the quality of life for patients with progressed, inoperable tumors (pancreas, biliary tract, colorectal, duodenal and oesophageal). Multivisceral oncological operations with vascular reconstruction if required are performed for patients with locally advanced neoplastic disease. For peritoneal carcinomatosis intraperitoneal hyperthermic chemotherapy is performed.

There are few trends in scientific research work in the Centre of Abdominal Surgery:

1. Hepatobiliary and pancreatic surgery. Advanced methods of hepatopancreatic surgery are introduced in the Centre: the first laparoscopic operations on biliary tract were performed; liver augmentation by embolisation and ligation of portal vein; treatment of hepatic malignant focal lesions using radiofrequency ablation; liver trisectorectomy with portal vein resection in patients with portal tumours; modern program of treatment of hepatic metastases. The first successful liver and pancreas transplantations were performed by the team of transplantologists, organised and headed by prof. K. Strupas. At present, the Centre of Abdominal Surgery of the Vilnius University Hospital Santaros Klinikos is undoubted leader in the Baltic States in the field of transplantation of abdominal organs. Lithuanian Echinococcosis registry is the first in the Baltic States as well as an integral part of the European Echinococcosis registry. After introduction of the new method of treatment of acute pancreatitis we achieved the results of the best European clinics in low mortality and complication rates. Advanced methods in visual and laboratory diagnostics of pancreatic diseases and minimally invasive surgery are widely introduced. The first in Lithuania preserving retroperitoneal endoscopic necrosectomy in patients with acute necrotic pancreatitis was performed in the Centre.

2. Coloproctology. Since 2003 the surgery on lower gastrointestinal tract has been performed mainly by minimally invasive methods and the Centre of Abdominal Surgery is the leader in the Baltic States in this field. Endoscopic transanal microsurgery is introduced for treatment of rectal polyps and early stage cancer. Colonic surgery is entirely performed laparoscopically by making minimal incisions in the abdominal wall – this guarantees patient comfort and early recovery. The Centre is the coordinator of the colonic cancer prevention program in Lithuania and in cooperation with gastroenterologists started a new method – colonoscopic elimination of minimal colonic formations under laparoscopic supervision. Single stage surgery in the case of colonic cancer complicated with obstruction is favoured, eliminating the need for enterostomy. A new specialized task scientific team was established for perineology and functional coloproctology employing both conventional and laparoscopic surgery.
3. Minimally invasive abdominal surgery. The Centre is among the institutions in Baltic States most actively implementing minimal invasive treatments and modern diagnostic and treatment technologies. Endoscopic and laparoscopic abdominal operations and intervention ultrasonoscopy are introduced and developed in the Centre. The Centre personnel participate in administration of Association of Lithuanian surgeons and Lithuanian Association of Minimally Invasive Surgery. It is must be mentioned the great experience in the morbid obesity surgical treatment held by the Centre.

4. Endocrine surgery. The standards of thyroid, parathyroid and adrenal gland surgery are under improvement, minimally invasive methods are introduced. Retroperitoneal endoscopic, single port retroperitoneal endoscopic and laparoscopic adrenalectomies and all other modern endocrine surgery operations are performed in the Centre.

5. Upper gastrointestinal tract surgery (The surgery of gastroduodenal ulcers). This is a strong trend in scientific work, continuing fundamental research in gastroduodenal surgery, started by prof. P. Norkūnas and prof. A. Bubnys. The Centre of Abdominal surgery is the main basis in East Lithuania region for the surgery of complicated esophageal and gastric diseases –esophageal achalasia and diverticulosis, gastroesophageal reflux disease, complicated ulcers. The new endoscopic methods for treatment of bleeding ulcers and small gastric tumours were introduced in cooperation with gastroenterologists.
