The main purpose of Centre of Anaesthesiology, Intensive Therapy and Pain Management (CAITPM )is - to provide the high quality, timely, adequate and safe care for  patients undergoing elective or urgent surgical and diagnostic procedures, and to carry out research in anaesthesiology, intensive care and pain therapy.

Ist Department of anaesthesiology and intensive care

Department provides anaesthesia, early postoperative care and pain management services in abdominal surgery, gynaecology, vascular surgery, urology, ear, nose and throat surgery, eye surgery for inpatient and day surgery patients, including anaesthesias for heart, kidney and liver transplantations. Unit also performs anaesthesia for various diagnostic procedures (magnetic resonance tomography, computed tomography) and low-invasive procedures (gastroscopy, colonoscopy). Department annually performs over 12,000 anaesthesias – for elective as well urgent procedures.
Anaesthesia is a pharmacologically induced and reversible state of amnesia, analgesia, loss of responsiveness, loss of skeletal muscle reflexes or decreased stress response, or all simultaneously. This allows patients to undergo surgery and other procedures without the distress and pain they would otherwise experience. A wide variety of drugs are used in modern anaesthetic practice, which are used only by trained staff and  using appropriate equipment and facilities to ensure the safety of the patient.
We regularly renew treatment stocks for extremely rare anaesthesia-related complications - malignant hyperpyrexia.
Acute pain service, which operates around the clock, is part of our department. This mobile team includes an anaesthesiologist, and anaesthesia-intensive care nurse. This enables high-quality service and therefore safe pain relief for patients after major operations.

Achievements of our department are apparent at the most complex and complicated cases – as organ transplantation- which requires a lot of cooperation between well-trained and competent surgeons, anesthesiologists, physicians and other team members. We can be proud of the transplantation success achieved in recent years.

In daily routine we pay attention to early mobilisation and quick return to everyday life and patient’s satisfaction by using short-acting anaesthetic drugs, effective postoperative pain treatment, prophylaxis of thrombosis and postoperative nausea.
We use modern and evidence based pharmaceutical and equipment tools to ensure patient safety and satisfaction.
In our department patients with extremely rare disorders are treated: pheochromocytoma, MEN syndrome, myasthenia gravis, neuroendocrine tumours, von Hippel-Lindau disease, eye coloboma, Ewing sarcoma.

Cancer patients receive modern treatments in our department: intraperitoneal hyperthermic chemoperfusion (HIPEC) is a type of hyperthermia therapy used in combination with surgery in the treatment of advanced abdominal cancers. In this procedure, warmed anti-cancer drugs are infused and circulated in the peritoneal cavity for a short period of time. This treatment is used for certain advanced stage tumours in the abdomen that have spread many small tumours throughout the abdomen, when surgery is not likely to be possible or effective. Our department started first HIPEC procedure in Lithuania and is continuing it use for severely ill patients.

IInd Department of anaesthesiology and intensive care

In our department are performed anaesthesias for cardiac surgery, and various interventions. During the year we perform 1500 anaesthesias, including 800 with cardiopulmonary bypass, 100 anaesthesias for complex electrophysiological procedures, and 30-40 anaesthesias for the modern hybrid operations during the transcatheter aortic valve implantation, atrial septal defect closure procedures as well as left ventricular aneurysm plication procedures.

The modern monitoring system (invasive pressures, cardiac hemodynamic parameters, cerebral oxymetry and depth of anesthesia) during anesthesia ensures consistent and objective tracking of organism vital functions. Anesthesiologists in our department are familiar with transesophageal echocardiography which is extremely valuable during cardiac operations.
Circulatory support devices (intraaortic counterpulsation, ECMO (extracorporeal membrane oxygenation ), VAD ( ventricular assist devices)) are widely used not only for cardiac surgery patients, but also for other acute cardiac and respiratory insufficiency states.

Ist Intensive Care Unit

Intensive care unit provides intensive care for 800-900 patients per year for patients suffering from respiratory, nervous system, after major surgery, including significant proportion of patients with severe sepsis and multiple organ dysfunction syndrome.  The treatment success depends on skilled doctors and advanced methods, applied in our department:
• Renal replacement therapy - haemofiltration, 
• Artificial detoxication devices as Molecular Adsorbents Recirculation System (MARS)
• Liver dialysis – bilirubin adsorption

Intensive care unit provides all types of modern treatment methods for critical care patients.
Early enteral nutrition, total parenteral nutrition is ordinary methods in our department.
All doctors are active members of Lithuanian Society of Enteral and Parenteral Nutrition.
Most nurses have a university education degree.

IInd Intensive Care Unit

Intensive care unit provides intensive care for 1700 patients with ischemic heart disease, pulmonary hypertension, patients after cardiac surgery, heart transplantation, complex electrophysiological, transcatheter aortic valve implantation, atrial septal defect closure procedures as well as left ventricular aneurysm plication procedures.

Continuous modern monitoring system of invasive pressure, hemodynamic, respiratory parameters, transesophageal echocardiography is widely used.
Advanced methods, applied in our department:

• Renal replacement therapy - haemofiltration,  haemodialysis
• Circulatory support devices (intraaortic counterpulsation, ECMO (extracorporeal membrane oxygenation ), VAD ( ventricular assist devices)) used not only for cardiac surgery patients, but also for other acute cardiac and respiratory insufficiency states
• Thromboelastometry – for evaluation of postoperative coagulation disorders

Department of paediatric anaesthesiology and intensive care

Anaesthesia and intensive care is given to children undergoing cardiac surgery, diagnostic and interventional procedures in the department. This is the only one in Lithuania, where the aid is given to newborns and infants. Over the years we perform about 200 heart operations. In 2009, in support of the Republic of Lithuania and Norway Grants Fund all facilities have been substantially updated. We argue that our current medical equipment in operating theatre and intensive therapy ward is one of the best in the world.
Some modern methods of treatment and monitoring we apply in our department:
1. Circulatory support by prolonged perfusion, extracorporeal membrane oxygenation, external artificial ventricles.
2. Treatment of pulmonary hypertension with sildenafil and NO gas.
3. High-frequency pulmonary ventilation.
4. Intravenous anaesthesia by TCI (Target Controlled Infusion) method in older children.
5. Monitoring of depth of anaesthesia (BIS), regional tissue saturation (NIRS), cardiac output (PICCO).
6. Modified ultra filtration
7. Early extubation after non complicated surgery.
Authorized persons have the opportunity to follow the condition of patients over the Internet Access (complete monitoring data with live view and trends, laboratory tests, X-ray, ultrasound images) regardless of their location.

Pain treatment day care department
Patients suffering a variety of persistent pain syndromes are consulted in Pain treatment day care department. A majority of them present with low back pain (~ 60) and joint pain (~ 30), other patients have neuropathic pain, pain after traumas and injuries, cancer pain.
Pain treatment day care department provides such services:
• Differential diagnosis
• Pain management
• Prescribing different type of pain medications
• Performing procedures or injections (trigger point injections, epidural injections, facet joint injections, nerve root blocks, radioablation therapy)
One of the options for difficult pain control cases are long term epidural catheters- catheter is placed into epidural space under sterile conditions. Later patient is trained to use epidural catheter, inject pain medicines.
Many procedures are performed using X-ray or ultrasound control, which increase the safety of the procedure.
We consult other health care providers (physical therapists, radiologists, neurologists, neurosurgeons, vascular surgeons) and involve the other specialist for multidisciplinary pain management approach.
Pain treatment day care department gives about 4,500 consultations and about 3,500 invasive pain management procedures during a year. 80-85 of patients are ambulatory patients, who come from all over Lithuania.
Pain treatment day care department gives training courses for different doctors from Lithuania as well the other countries.

Over the past few years some new procedures were introduced:  thoracic sympathectomy for neuropathic and ischemic chest and arm pain; neurolysis of solar plexus and splanchnic plexus for chronic pancreatitic pain, controlled epidural catheter for low back pain (these procedures are useful in patients after spinal surgery). We participate in the selection of patients for intrathecal baclofen therapy, as well as the baclofen pump implantation.
