In the latter years research trends of the Centre are more directed towards studies on chronic liver disease pathogenesis with other damages of abdominal organs and optimal combination of therapeutic and surgical treatment.

One of the significant achievements of the Centre is adaptation of methods of diagnostic research and treatment in medical practice (fundamental immunological, morphological and virusological diagnostic); quite time taking monitoring of the patients, observation and treatment system is designed with the crucial purpose to stop progressive chronicle liver diseases and to avoid complications and to assign modern medical help. At the Centre specialists are observing patients before and after liver transplantation.

Since 1991, the Centre has been actively developing pedagogical activity which includes teaching students and residents of the 5th-6th, lately the 4th years courses and improving the qualification of physicians in the fields of gastroenterology (tutor prof. A. Irnius), hepatology (tutor assoc. prof. D. Speiciene), dietetics (tutor dr. E. Gaveliene), abdominal organs’ sonoscopy (tutor prof. J. Valantinas), gastroenterological pharmacology, functional pathology of digestion and nonmedicamental therapy of diseases. Every year about 150 students enrolled in the Centre and about 100 physicians improve their skills during the courses. Majority of gastroenterological oriented courses are prepared and held in the Centre. After 1991 about 1500 physicians improved their skills during the various gastroenterological courses. Since 1992 a medical residency is in place that satisfies EU requirements; physicians’ gastroenterologists are being trained towards their qualifications. Several dozen of specialists successfully finished their residency years. The scientific research in the Centre is provided by gastroenterology PhD. There are 4 ongoing projects at the moment. Since the existence of the Centre over 30 study books and other glossary materials were published by the Centre pedagogues.

Every year around 2100 patients are treated in the Departments of Hepatology and Gastroenterology. Over 200 liver biopsies; 50 pancreas and other organ puncture biopsies; more than 100 abdominal ascites punction diagnosed and treated every year.

The following performed in the Centre:

  • Treatment of gastroesophagic reflux with proton pump inhibitors, H2 receptor antagonists and prokinetics;
  • Treatment of complicated stomach ulcer (detection the level of the H. pylori infection, bleeding, stenosis);
  • Diagnostic and treatment of ulcerative colitis and Crohn’s disease (with mesalamine, glucocorticoids, immunomodulators, serious forms of Crohn’s disease with biological medication);
  • Dietary treatment of enteropathies;
  • Correction of bacteria excess syndrome;
  • Diagnostic of pancreas diseases and complications (application of pancreas puncture biopsy, endoscopic retrograde cholangiopancreatography, computed tomography, magnetic nuclear resonance in diagnosis of pancreas disorders) and treatment;
  • The newest methods of diagnosis and assessment of lever diseases: examination of biochemical, immune (indices of cellular and humoral immunity, autoantibodies), virological (detection of viral load and genotypes of HCV and HBV, CMV infection, Epstein-Bar, etc. genotype result and viraemia level), tumor markers, bloodstream of hepar diagnosed using modern sonoscopy methods, biopsy of focal hepar lesions, histological examination and relevant treatment in agreement with codent requirements.
  • Modern echinococosis diagnosis and treatment;
  • Test of ascite liquid by means of biochemical, microscopical and bacteriological examination, diagnosis of spontaneous bacterial peritonitis, long-term drenage of peritonem;
  • Transjugular liver biopsies;
  • Treatment of chronic hepatitis B and C and liver cirrhosis with antiviral medicine;
  • Treatment of autoimmune hepatitis and other autoimmune liver diseases (primary biliary cirrhosis and primary sclerosing cholangitis) by means of modern immunosuppressive drugs and ursodeoxycholic acid;
  • Local treatment of tumorous foci, liver damages and liver cysts by injecting medicine or substances;
  • Diagnosis of latent hepatic encephalopathy using psychometric tests;
  • Diagnosis and specific treatment of metabolic diseases (hemochromatosis, Wilson’s disease, porphyria, etc.);
  • Selection and observation of patients before and after liver transplantation;
  • Application of enteral and parenteral nutrition for the treatment of patients with various digestive diseases;
  • Modified dietetic treatment of the digestive system diseases.

Endoscopic diagnosis and minimally invasive surgery department perform per year:

1. about 7000 diagnostic Esophagoscopy Gastroscopy Duodenoscopy (EGD). During the EGD intervention:

-          polipectomia (40);

-          mucosectomy (endoscopic submucosal dissection) (15);

-          control of bleeding (170 – 200);

-          endoscopic sonoscopy (80 with 20 fine needle aspiration (FNA));

-          gastroduodenal stenting with self-expanding metal stents (5);

-          feeding tube records (110);

-          percutaneous endoscopic gastrostomy (PEG) (10);

-          endoscopic stenting – 50/per year;

2. about 2600 diagnostic colonoscopy. During the colonoscopy intervention:

-          polipectomia (700);

-          control of bleeding (10);

-          argon plasma coagulation (10);

-          stenting (5);

3. about 500 endoscopic retrograde cholangiopancreatography:

-          stenting plastic stents (100);

-          stenting with self-expanding metal stents (20);

-          papilosfincterotomia (150);

-          stones from the bile and pancreatic duct removal (170);

-          polipectomia (3);

  1. The department is involved in colorectal cancer screening program.
  2. Various interventional procedures by endoscopic control (fine-needle biopsy, pancreatic pseudocysts drainage) and endoscopic tests are performed. Endosonoscopy test is used to determine a stomach, esophagus, pancreas, gall bladder and bile ducts tumor stages; stomach and esophageal subepithelium derivatives diagnostic.
  3. Since 2007 the department perform a stomach endoscopic submucosal dissection (mucosectomy type). Pathological change in a stomach (it could even be an early stage of a gastric cancer without evidence of spread) could be eliminated before a muscle tissue is consumed by injecting solutions and specific modified scalpels. Such procedure also performs on a colon and a rectum in the future.
  4. Since 2006 endoscopic esophageal varicose veins ligation of rubber rings in patients with liver cirrhosis is performed.
  5. Most intervention procedures are performed in VII surgical block which part of the Endoscopic diagnosis and minimally invasive surgery department: endoscopic retrograde cholangiopancreatography (stones removal from a bile and pancreatic duct, complex pancreatic and biliary stenting); gastrointestinal stenting, extension procedures; complex polipectomia, mucosectomy; percutaneous endoscopic gastrostomy; endoscopic sonoscopy inquiryies.
  6. For over 4 years department uses stenting technique with self-expanding metal stents of bile duct, stomach-duodenum, colon and esophageal.
  7. Since 2008 considerable experience in treating patients with chronic pancreatitis has accumulated.
  8. The department use a high-resolution endoscopes with narrow band imaging (NBI) technology which allows to identify early tumor lesions in gastrointestinal mucosa. It is planned to apply a chromoendoscopy with indigo carmine,lugol, methylene blue and other coloring agents.
  9. The department perform an enteroscopy (small bowel endoscopy) and direct bile duct endoscopy (with biopsy and direct lithotripsy).

Every year the 2nd department of Gastroenterology provides diagnostic and treatment to 700 patients suffering from digestive diseases and disorders. Modern gastroenterological diseases diagnostic and treatment methods performed
